Arrival Time
Please arrive 15 minutes before your scheduled appointment to complete a questionnaire/registration form. You are welcome to bring someone with you to your appointment.

What to Bring
Federal and state regulations require that we verify your identity with photo identification and check your address. Patients over the age of 18 will be asked to show the following forms of identification:
- Photo ID (i.e. driver's license, passport)
- Identification card that verifies your address (i.e. driver's license)
- Health insurance card (i.e. HMO, PPO, Medicare, Medi-Cal, etc. card) if insured
We also recommend that you carry a listing of your current medications, dose and frequency. If you're a new patient, please bring your previous doctor's information (name, address, phone number). Check with your insurance company, as you may be required to pay your deductible or co-payment.

Registration Information and Online Registration Form
Prior to your visit, you may be asked to complete a registration form and bring it to your appointment. You may also elect to complete our online registration form.
Please note that online registration may take a few days for us to process, so if your appointment is within the next few days, please complete the paper form prior to the appointment or arrive 15 minutes before your scheduled appointment to complete the form.
You may also call 855-604-3224 and a call agent will assist you in completing the registration over the phone. We know your time is valuable. This questionnaire helps in the efficiency of your evaluation and assures that the physician and medical team have a comprehensive record of your health status.

Late Arrivals and Cancellations
If you arrive more than 15 minutes late, you may have to wait until the doctor has seen other patients who were on time. We make every effort to try and accommodate all of our patients, however, you may be asked to reschedule your appointment for another day.
If you need to cancel your appointment, please call your doctor’s office or 855-604-3224 as soon as possible or no less than 24 hours in advance to reschedule.

Insurance Card
If you have HMO/Managed care coverage, review your insurance card to make sure Sutter East Bay Medical Foundation is identified as your primary medical group. If not, please contact your insurance company’s member services department to ensure your plan gives you access to this Sutter doctor prior to completing the registration form.
After we receive your registration form we will verify your insurance coverage and contact you if we have any questions. If you like to speak with someone in person, you may also call 855-604-3224 and a call agent will assist you in completing the registration over the phone.
